Chronic Disease Management FAQ
In version 9.12 of Practice Manager, we have an easy way to list patients that have not been billed for Chronic Disease or Complex Care incentives. You can do the same for patients eligible for complex care billing incentives. These features of Practice Manager can save you time, effort and money. Some clinics have saved enough to more than cover the costs of implementing an EMR system in their office. Contact us if you'd like to know more about this feature or to find out about upgrading to the latest version of Practice Manager.
To generate a list of patients who are fully billed or not fully billed follow these steps:
- From the main menu, select Patients, Complex Care/Chronic Disease to display the Complex Care/Chronic Disease Patient screen.
- Select Patient List with Billing Incentive Information as the Report Type.
- Select the Report Category you wish to produce by selecting Complex Care, Chronic Disease or Both.
If you choose Complex Care you will generate a list of complex care patients. If you select Chronic Disease the list will include patients who qualify for CDM incentive fees. Patients will be included in the list based on the conditions they have been diagnosed with in their chart and any past billings they have with applicable diagnostic codes.
- Choose the Billing Status you wish to report on. Choose Fully Billed to produce a list of patients who have been fully billed. Choose Not Fully Billed to generate a list of patients for whom additional billing can be done.
- In the Filter By Registered Doctor field, select a doctor if you want to include only patients registered to a particular doctor or choose All Doctors to run the report for all doctors in the practice.
- Press Search to start the search for patients.
- When the search is complete, the list of patients is shown.

From the main screen go to Reports, Patients by Condition.

- Enter the diagnosis code you are searching for and press the > button to add it to the list.
- You can enter multiple codes, pressing the > button after each one.
- ClickOK and it will list the patients that have the appropriate codes listed as a condition in their chart.
If you are not already using the CDM flowsheets in the Encounters section of their chart, you should use these to record their fowsheet information. This will make tracking and managing your Chronic Disease patients much easier.
- Open the chart that you want to search, or any chart if you want to search your whole system.

In the icons across the top of the chart window you will see Search with a little magnifying glass symbol next to it.

- In the dialog box enter the text you want to find, e.g. CKD.
- Select this patient to just search the current chart, or ALL patients to search all patient charts.

If you are already using the EMR CDM encounter flowsheet features, the CDM Patient List report can be generated quickly with the following steps.
NOTE: If you are not yet using the EMR CDM encounters, not all the data for the patient lists is available in the system. To find out how to identify CDM patients read this FAQ item "How do I list CDM or complex care patients with unbilled incentive fees". We recommend that you use the EMR CDM encounters to save time during the encounter and save time when you would like reports on your CDM patient population.
In the main screen select Reports, CDM Patient Lists.

- Select the Doctor code from the drop-down list.
- Select then the condition you want to search for (or All Conditions for a complete list).
- Click OK and it will list all patients that have encounters in their chart for the appropriate Chronic Disease.
If you are already using the EMR CDM encounter flowsheet features, the Key Measures report can be generated quickly with the following steps.
NOTE: If you are not yet using the EMR CDM encounters, not all the data for the key measures is recorded in the system so a report cannot be generated easily. We recommend that you use the EMR CDM encounters to save time during the encounter and save time when you would like reports on your CDM patient population.

In the main screen, select Reports, CDM key Measures.

- Pick the required Doctor Code and CDM flowsheet type (for example Diabetes).
- In the Start Date, enter today’s date and in the Report Type select your preferred report.
- Click OK and you will see all of the key measures for the specific condition.

If you are already using the EMR CDM encounter flowsheet features, the CDM Recalls report can be generated quickly with the following steps.
NOTE: If you are not yet using the EMR CDM encounters, not all the data for the recalls is recorded in the system so a report cannot be generated easily. We recommend that you use the EMR CDM encounters to save time during the encounter and save time when you would like reports on your CDM patient population.
In the main screen, select Reports, CDM Recalls.

- Pick the required Doctor code and the CDM flowsheet for which you want to see the recall status.
- Click OK and it will show all patients for the selected condition. A number in the box indicates the number of months until the patient is due for the recall, an X indicates that the patient is overdue.
